Mounjaro® is FDA-approved for type 2 diabetes, while Zepbound® is approved for weight management in eligible individuals. Engaging two incretin pathways gives tirzepatide a different metabolic profile from semaglutide. It can reduce appetite and food intake while improving glucose regulation. It is also gradually titrated over time.

Semaglutide
Semaglutide has produced substantial weight reduction in major clinical trials. In the STEP 1 trial, adults with overweight or obesity without diabetes receiving once-weekly semaglutide 2.4 mg alongside lifestyle intervention lost approximately 14.9% of their initial body weight on average at 68 weeks, compared with 2.4% with placebo. Tirzepatide
Tirzepatide has pushed average reductions further. Side effect profile
Semaglutide
The most common semaglutide side effects are gastrointestinal. You may experience nausea, vomiting, diarrhea, constipation, abdominal discomfort, or indigestion, especially while beginning treatment or increasing your dose. However, many of these side effects can be managed by gradually increasing dosage over time.
Tirzepatide
Tirzepatide commonly causes similar gastrointestinal effects, including nausea, diarrhea, constipation, vomiting, indigestion, and abdominal discomfort. Some clients tolerate it extremely well, and others do not. Gradual titration can help with some of the side effects. The art lies in finding the appropriate intervention and dose for the individual.
Contraindications
Semaglutide
Semaglutide should not be used if you have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Your provider also needs to understand your broader medical history and medications. Other potential risks may include pancreatitis, gallbladder disease, and acute kidney injury due to dehydration.
Tirzepatide
Tirzepatide is contraindicated with a personal or family history of MTC or MEN 2. Your provider should also evaluate your medications, gastrointestinal history, pancreatic and gallbladder concerns, kidney health, and other relevant risk factors. Tirzepatide can also affect the absorption of oral medications because it delays gastric emptying.

Semaglutide
Semaglutide’s benefits can extend beyond weight reduction. Wegovy is FDA-approved to reduce the risk of cardiovascular death, heart attack, and stroke in adults with established cardiovascular disease and either obesity or overweight.
